First/Lost/Stolen
- Application form completed in Black Ink (Written
or Printed)
- Original birth certificate, (if birth certificate is not
older than 18 months old, 2 forms of ID will be required.
Except for babies.
- 2 passport size photos (white background, no more than six
months old).
- The form will need to be countersigned by a professional
person
- Examples of professional persons: - Doctor, Dentist, Solicitor,
Barrister, Director of Ltd company, Pharmacist who has a degree,
MP, Judge, Police Officer, Nurse or Civil Servant.
- 1 of the passport sized photos will need to be certified
by the same professional person with the statement "I
certify that this is a true likeness of" (applicants
name), sign and date.
- The counter signatory must have known the applicant a minimum
of 2 years and they must be a British Citizen and not a relative.
The contact number given must be a land line, mobile numbers
are not accepted.
- If child's name is on mother or fathers passport, this will
need to be supplied with the application so the child's name
can be removed.
- Proof of travel.
Second Passport
- Original application form completed in Black Ink
(Written or Printed)
- 2 passport size photos (white background, no more than six
months old).
- Photocopy of passport
- Detailed company letter stating the reason for requiring
a second passport and the letter must state the countries
that the applicant will be travelling to and especially the
countries that will be visited that require visas.
- The form will need to be countersigned by a professional
person.
- 1 of the passport sized photos will need to be certified
by the same professional person with the statement "I
certify that this is a true likeness of" (applicants
name), sign and date.
- Proof of trips; i.e. itinerary for the next 6 months.
IN ALL CASES DETAILS OF ALL PASSPORTS WILL NEED TO BE SUPPLIED
Passport Renewals
- Original application form completed in Black Ink.
- 2 passport size photos (white background, no more than six
months old).
- Passport (expired or due to expire).
- Application form and photos will only need to be countersigned
if the applicant has changed in their appearance from the
photo in the old passport.
- Proof of travel.
Passport Extensions
- Original application form completed in Black Ink.
- 2 passport size photos (white background, no more than six
months old).
- Passport to be extended.
- Form will need to be countersigned by professional person
and one of the photos will also need to be certified by the
same professional person.
- Proof of travel.
Passport photo to be changed
- Original application form completed in Black Ink.
- Passport
- 2 passport size photos (white background, no more than six
months old).
- Marriage Certificate/Divorce papers/Deed Poll documents.
- Proof of travel.
- The application form will need to be countersigned by a
professional person plus one of the photos will need to be
certified by the same professional person.
